At Transitions Care we rely on the support from our volunteers to share their time and talents to provide the highest level of care. We believe volunteers are a vital part to the patient care team as they provide comfort and companionship to the patient like no other. We have several volunteer opportunities to choose from that can be tailored to fit your busy schedule.Our hospice companion volunteers visit with our patients a few times a month. These visits provide companionship to our patients and for patients living at home our volunteers provide much needed relief to primary caregivers to take some time for self-care. Common activities our volunteers do with our patients include:Reading to the patientSharing storiesSinging to the patientSharing storiesTaking walks or just getting the patient outside on a nice dayPlaying gamesHome patients can sometimes use additional support including light house work, meal prep, or yardwork
