Assemble Care Packages for NICU Families

Registration must be done PER PERSON, including children. Volunteers will be assembling smaller items that will go into our Packages of Hope. Items that volunteers could be packing are paper plates, cups, napkins, utensils, bowls, hand sanitizer, hand soap, paper towels, bottles, pacifiers, baby care tools, coloring books, toys, crayons, etc.
By signing up for this event, you are agreeing to Lily's Hope Foundations VOLUNTEER ACKNOWLEDGMENT AND AGREEMENT
I acknowledge that
I am volunteering with Lily's Hope Foundation voluntarily and understand that I am not an employee and will receive no compensation or benefits.
I assume all risks associated with my volunteer participation and release Lily's Hope Foundation, its officers, directors, employees, agents, and volunteers from liability arising from my participation, to the fullest extent permitted by Pennsylvania law.
I understand that Lily's Hope Foundation does not provide medical, health, disability, or liability insurance for volunteers.
In the event of an emergency, I authorize emergency medical treatment if necessary and understand that I am responsible for any resulting expenses.
I understand that I may become aware of personal, family, financial, or health-related information regarding families served by Lily's Hope Foundation and agree to maintain the confidentiality of such information.
I understand that Lily's Hope Foundation does not provide medical advice, and I will not provide medical recommendations or treatment guidance to families.
I grant Lily's Hope Foundation permission to use photographs, video, or audio recordings of me taken during volunteer activities for educational, promotional, fundraising, website, and social media purposes.
