Weekly Patient Support Call Volunteer

Provide weekly phone calls to home hospice patients to assess symptom management and identify medication or supply needs before the weekend. Calls are typically completed on Thursdays and may be made from the comfort of your home or from the office, based on your preference.
During each call, volunteers:
- Check on the patient's overall well-being and symptom management.
- Identify any medication, medical supply, or service needs.
- Answer questions and provide compassionate support within volunteer guidelines.
- Document all identified needs, concerns, and questions.
- Forward documentation to the Volunteer Coordinator for follow-up and appropriate action.
