Care Coordination and Outreach Services at MSM

Madison Street Medicine (MSM) provides street-based medical outreach and care coordination for individuals experiencing homelessness and housing instability. Through volunteer-driven care and strong community partnerships, MSM works to improve access to healthcare and connect individuals to ongoing services that support long-term stability. MSM continues to expand its reach and is seeking to strengthen core capacity-building systems to support sustainability and growth. This includes formalizing policies and procedures, improving communication systems, strengthening volunteer and community engagement, and enhancing resource development efforts.
